What will happen on my first visit?

Dr. Michaelides will complete a comprehensive medical and dental history and a thorough examination. Digital X-rays and photographs may be taken if necessary. After the exam we will be able to diagnose your periodontal and/or dental implant needs and suggest the best treatment to meet your needs and desires. Please allow 90 minutes for this appointment.

Please assist us at the time of your initial visit to the office by providing the following information:

• Bring your referral slip and x-rays from your referring dentist
• Fill out the New Patient Form, including the medications you are currently taking (we will mail you the form or you can download it from our website).
• Bring your insurance card and ID with you to the appointment.

All patients under 18 must be accompanied by a parent or guardian.
